- Administer medications and injections accurately following HIPAA guidelines and aseptic techniques to ensure patient safety.
- Manage complex clinical procedures such as IV infusion, catheterization, and wound care with precision.
- Coordinate discharge planning and collaborate with case management teams to facilitate smooth transitions of care across settings.
- Support infection control protocols and maintain sterile processing standards to prevent healthcare-associated infections.
- Engage in patient education on health coaching topics like nutrition, medication management, or chronic disease management.
Qualifications
- Valid RN license with current registration in the applicable state or jurisdiction.
- Proven experience working with diverse patient populations including long-term care residents, individuals with developmental disabilities, or those requiring memory care.
- Demonstrated proficiency with EMR/EHR system Kinnser/Wellsky accurate documentation and coding (ICD-10/ICD-9).
- Skilled in performing advanced clinical procedures such as IV insertion, phlebotomy, lab specimen collection/processing, and tube feeding management.
- Ability to work effectively and independently while maintaining high standards of patient safety and care quality.
- Excellent communication skills with the ability to collaborate across multidisciplinary teams including physicians, therapists, and case managers.
