Key Responsibilities
- Communication Excellence: You'll be a central point of contact, ensuring seamless communication within the rehabilitation department and with other departments, administration, and external partners. - Program Facilitation & Development: You will be instrumental in the development, implementation, and evaluation of new and existing rehabilitation programs that optimize patient outcomes and meet regulatory standards.

Additional Responsibilities
- Conduct comprehensive evaluations and develop individualized treatment plans for patients with a wide range of conditions
Assumes the duties of the Director of Rehabilitation in absence of the Director
Assists with scheduling of patient treatments
Assists with education and in-service training when needed
Compiles and submits requisitions for supplies and equipment
MDS completion for rehab items
Leads departmental meetings (in Director’s absence) and attends inter-departmental meetings when appropriate
Qualifications
NYS Licensed Physical Therapist or Occupational Therapist
Experience in SNF setting preferred
Experience with Net Health Therapy software preferred
MDS and PDPM knowledge preferred
