At Valley Mountain Regional Center, we're looking for compassionate, dedicated professionals to join our team as Service Coordinators. This rewarding role offers the opportunity to directly impact the lives of individuals with developmental disabilities while building a fulfilling career in human services. This role may support either Children or Transition/Adult programs depending on assignment.
We’re Hiring at All Locations:
- Stockton
- Modesto
- Tracy
- San Andreas (serving Tuolumne, Amador, and Calaveras counties)
- Note for San Andreas applicants: Your caseload location will be based on where you live (Tuolumne, Amador, or Calaveras).
What You’ll Do:
- Be a Champion for Others
- Advocate for the rights and services of individuals with developmental disabilities. Empower people and families to navigate systems and access life-changing support.
- Connect People to Resources
- Coordinate essential services such as healthcare, education, housing, and vocational programs that improve quality of life.
- Build Meaningful Relationships
- Work directly with individuals, families, schools, and community partners to promote independence and inclusion.
Key Responsibilities:
- Case Management & Planning
- Develop, coordinate, and monitor Individual Program Plans (IPPs)
- Facilitate team meetings and periodic reviews
- Documentation & Reporting
- Maintain detailed and timely case notes
- Complete evaluations, service authorizations, and Medicaid documentation
- Advocacy & Support
- Provide education, advocacy, and crisis support
- Act as liaison between clients, providers, and agencies
- Professional Collaboration
- Participate in committees and cross-agency initiatives
- Maintain positive relationships with service partners
- Emergency Response
- Serve as Officer of the Day and participate in After-Hours Response rotation
- Data & Analysis
- Collect and report data for surveys, assessments, and audits

What You’ll Bring/Qualifications:
- Bachelor’s degree in Human Services or related field and 1 year related experience
OR Bachelor’s in other field with 3 years of experience providing services to individuals with developmental disabilities - Valid driver’s license and reliable transportation
- Commitment to advocacy and person-centered services
