At Valley Mountain Regional Center, we're looking for compassionate, dedicated professionals to join our team as Service Coordinators. We are hiring for all office locations:
- Stockton
- Modesto
- Tracy
- San Andreas (serving Tuolumne, Amador, and Calaveras counties)
- Note for San Andreas office: Your caseload location (Tuolumne, Amador, or Calaveras) will be based on where you reside.
What You'll Do:
- Champion for Others: Advocate for the rights and needs of individuals with developmental disabilities
- Connect People to Resources: Coordinate essential services that enhance quality of life
- Build Meaningful Relationships: Work directly with consumers, families, and community partners
Key Duties for the Service Coordinator Position:
- Assess, coordinate, and implement individual program plans through team processes
- Schedule and facilitate various reviews and team meetings
- Complete comprehensive case documentation including program plans, evaluations, progress reports
- Prepare service authorizations, court reports, and Medicaid documentation
- Provide advocacy, education, and crisis intervention for clients and families
- Serve as liaison between clients and various service providers
- Collect and report client statistical data for various surveys and assessments
- Participate in internal/external committees for education and agency improvement
- Maintain positive communication with partner organizations
- Serve as Officer of the Day and participate in After Hours Response System as assigned
Areas of Support
This position may support individuals in the Children or Transition/Adults programs, depending on assignment and organizational needs.

Qualifications:Position Requirements:
- Bachelor's degree in Human Services or related field and (1) year related experience OR Bachelor's degree in a non human service related field and (3) years related experience providing training or direct services to persons with developmental disabilities in a setting such as day program, special education, or residential setting
AND
- Valid driver's license and reliable transportation
- Passion for advocacy and supporting individuals with developmental disabilities
