Title: Outreach Coordinator
- Status: Full-time, nonexempt, in-person
- Hourly rate: $23-$25
- Schedule: Sunday - Thursday, 9:00am - 5:30pm
- Location: Downtown San Diego
Responsibilities:
- Conducts phone screens to assess initial eligibility and immediate needs
- Monitors and tracks all incoming referrals from partner agencies, shelter networks, and outreach teams
- Keeps a detailed and organized referral log, ensuring accurate documentation, timely follow-up, and coordination with program staff
- Conduct direct street outreach in targeted areas
- Build rapport with youth experiencing homelessness using person-centered, harm-reduction practices
- Provide crisis intervention, de-escalation, safety planning, and basic needs supplies (food, hygiene packs, clothing)
- Offer information about program eligibility, available shelter beds, and community resources
- Complete outreach logs, encounter forms, and HMIS data entry as required
- Screen youth for eligibility (ages 1825, experiencing homelessness, safety considerations)
- Coordinate intakes and warm handoffs into the TAY shelter program
- Communicate with Program Manager and Residential Services Coordinators regarding available beds and same-day placements
- Assist youth in obtaining IDs, Medi-Cal, CalFresh, and other mainstream benefits
- Work in coordination with County, RTFH, PATH, YMCA, SDYS, YAC, and other TAY-serving organizations
- Maintain regular presence at community events, youth drop-ins, food distributions, school campuses, and outreach collaboratives
- Strengthen referral pathways with hospitals, behavioral health providers, law enforcement, County agencies, and shelters
- Represent the organization in outreach meetings, collaborative workgroups, and community networks
- Provide basic case support during outreach (crisis assessment, safety checks, housing triage)
- Link youth to behavioral health services, substance use treatment, job programs, and education opportunities
- Support with transportation coordination for shelter entry, appointments, and urgent needs
- Follow trauma-informed, culturally responsive, LGBTQ-affirming practices
- Complete accurate HMIS entries, outreach logs, shift summaries, and incident reports
- Maintain confidentiality per HIPAA, HMIS, and agency policy
- Track youth engagement, referrals, and outcomes for reporting
- 12 years of experience working with youth, homeless services, behavioral health, outreach, housing, or crisis services (lived experience encouraged)
- Knowledge of homelessness systems in San Diego, community resources, and TAY-specific needs
- Strong skills in crisis intervention, rapport-building, harm reduction, and de-escalation
- Ability to work independently in the field and maintain professionalism in unpredictable situations
- Valid California drivers license, reliable transportation
- Ability to pass background check (per agency requirements)

- Ability to travel between Urban Street Angels locations
- Ability to lift up to 20 lbs. and work in varied indoor/outdoor settings
- Flexibility to work evenings or weekends as needed
- Must submit to a background check via Live Scan (with ongoing alerts), as required by specific programs, funding requirements and licensing regulations
- Must submit to TB testing
- Must be authorized to work in the U.S. and complete I9 verification
