Urban Street Angels has made it our mission to serve this under-supported group, and in so doing, make meaningful progress toward ending homelessness in San Diego and beyond.Title: Outreach CoordinatorStatus: Full-time, nonexempt, in-personHourly rate: $23-$25Schedule: Sunday - Thursday, 9:00am - 5:30pmLocation: Downtown San DiegoPosition Summary: The Outreach Coordinator is responsible for engaging youth experiencing homelessness (ages 1825) across San Diego County and connecting them to Urban Street Angels Behavioral Health Bridge Housing (BHBH) program, support services, and community resources. This role conducts field-based outreach, builds relationships with youth in crisis, coordinates referrals with community partners, and ensures warm handoffs into the BHBH program and supportive services. The Outreach Coordinator promotes a trauma-informed, Housing First, youth-centered approach in all interactions.Responsibilities: Conducts phone screens to assess initial eligibility and immediate needsMonitors and tracks all incoming referrals from partner agencies, shelter networks, and outreach teamsKeeps a detailed and organized referral log, ensuring accurate documentation, timely follow-up, and coordination with program staffConduct direct street outreach in targeted areasBuild rapport with youth experiencing homelessness using person-centered, harm-reduction practicesProvide crisis intervention, de-escalation, safety planning, and basic needs supplies (food, hygiene packs, clothing)Offer information about program eligibility, available shelter beds, and community resourcesComplete outreach logs, encounter forms, and HMIS data entry as requiredScreen youth for eligibility (ages 1825, experiencing homelessness, safety considerations)Coordinate intakes and warm handoffs into the TAY shelter programCommunicate with Program Manager and Residential Services Coordinators regarding available beds and same-day placementsAssist youth in obtaining IDs, Medi-Cal, CalFresh, and other mainstream benefitsWork in coordination with County, RTFH, PATH, YMCA, SDYS, YAC, and other TAY-serving organizationsMaintain regular presence at community events, youth drop-ins, food distributions, school campuses, and outreach collaborativesStrengthen referral pathways with hospitals, behavioral health providers, law enforcement, County agencies, and sheltersRepresent the organization in outreach meetings, collaborative workgroups, and community networksProvide basic case support during outreach (crisis assessment, safety checks, housing triage)Link youth to behavioral health services, substance use treatment, job programs, and education opportunitiesSupport with transportation coordination for shelter entry, appointments, and urgent needsFollow trauma-informed, culturally responsive, LGBTQ-affirming practicesComplete accurate HMIS entries, outreach logs, shift summaries, and incident reportsMaintain confidentiality per HIPAA, HMIS, and agency policyTrack youth engagement, referrals, and outcomes for reportingQualifications: 12 years of experience working with youth, homeless services, behavioral health, outreach, housing, or crisis services (lived experience encouraged)Knowledge of homelessness systems in San Diego, community resources, and TAY-specific needsStrong skills in crisis intervention, rapport-building, harm reduction, and de-escalationAbility to work independently in the field and maintain professionalism in unpredictable situationsValid California drivers license, reliable transportationAbility to pass background check (per agency requirements)Additional Requirements:Reliable transportationAbility to travel between Urban Street Angels locationsAbility to lift up to 20 lbs. and work in varied indoor/outdoor settingsFlexibility to work evenings or weekends as neededMust submit to a background check via Live Scan (with ongoing alerts), as required by specific programs, funding requirements and licensing regulationsMust submit to TB testingMust be authorized to work in the U.S. and complete I9 verificationRelevant evidence-based practices and leading outcome-driven careBenefits:Urban Street Angels offers an array of benefits to employees, supporting health, wellness and retirement, including: 401(k) retirement savingsMedical insuranceDental insuranceVision insuranceAncillary voluntary insurancesPaid time off including sick and vacation timeReferral incentive programUrban Street Angels is an equal opportunity employer, committed to creating a diverse and inclusive workplace.
