Job Summary:
Under the supervision of the Assistant Director of Wellbeing, the Assistant Manager of Medical Services will be responsible for the support of clinical operations via a variety of clerical and patient services, necessary to maintain efficient workflow in the medical clinic, as well as patient care. The support of operations will be established through care coordination support for other medical staff, including providers and collaboration with all agency programs. This position will represent Urban Peak and its medical clinic services to enhance and promote our agency mission to ignite the potential in youth to exit homelessness and create self-determined and fulfilled lives. This position will assist in providing quality patient-centered care, always exercising utmost discretion, diplomacy, tact, and adhering to HIPAA regulations during youth interactions and with community partners. Duties include care coordination, QMAP oversight, and administrative support.
This position will ensure that interactions, interventions, and environments at Urban Peak are trauma-informed and emphasize the physical and emotional safety of youth and staff.
Who You Are:
- A bachelor’s degree in counseling, social work, psychology, or a related science/health field AND two (2) years’ experience working with youth/adolescents OR people experiencing houselessness OR Medical Assistant Certification from a Committee on Accreditation of Allied Health Education Programs (CAAHEP) accredited college AND minimum of two (2) years of experience working with youth or related experience; or an equivalent combination of education and experience.

- Preferred Experience working as a Care Coordinator, Case Manager [in a medical clinic setting], Patient Navigator, and/or Medical Assistant
- Lived experience reflective of the youth we serve encouraged but not required
- Must be computer proficient and possess experience of Microsoft Office Suite (e.g. Outlook, Word, PowerPoint, and Excel)
- Understanding of the provision of and commitment to trauma-informed care and positive youth development
- Must be insurable as a driver on Urban Peak’s auto liability policy in order to conduct business on behalf of the agency or transport youth using an Urban Peak vehicle and/or an approved personal vehicle
- Ability to use second language in written and verbal communication desired (Spanish, Vietnamese, ASL)
- Must pass both federal and state background checks
- Must pass pre-employment physical
- Must pass pre-employment tuberculosis (TB) screening
- Provide proof of education (HS Diploma OR Official Transcripts)
What You Will Do:
Care Coordination (70%)
- Coordinates care with community providers and other providers within the clinic to ensure youth patient needs are being supported
- Connects and empowers patients to access identified resources and assists in navigating systems of specialty care to meet identified needs
- Communicates appropriate clinical information as needed to complete referrals in a timely manner
- Works from a strengths-based, harm reduction perspective, and takes cultural considerations into account
- Assists in grant reporting and deliverables
- Responsible for accurate data collection and documentation
- Obtaining healthcare documentation for youth: including, but not limited to, Releases of Information (ROI), discharge paperwork, medical records, medical face sheets, etc.
- Helping maintain and build relationships with new and existing community partners
QMAP Oversight (20%)
- Providing or assisting with in-house QMAP training
- Weekly auditing of documentation and QMAP errors, in tandem with other supporting staff
- Assisting in the write up of QMAP error narratives
- Aiding and/or maintaining accountability regarding weekly medication counts
- Communication between providers, Urban Peak medical clinic, and case managers for coordination of care.
- Creating MAR and profile sheets and supporting clients in medication refills and adherence to Urban Peak’s medication compliance policies.
- Maintenance of QMAP policies and documentation.
Administrative Support (10%)
- Quality control of digital and physical medical documentation and maintenance of clinical information and patient records
- Maintenance of clinical supply inventory and assistance with ordering.
- Other duties as assigned.

Who We Are:
Founded in 1988, Urban Peak is the only non-profit organization in Denver that provides a full convergence of services for youth ages 12 through 24 experiencing homelessness or at imminent risk of becoming homeless. Our goal is to meet youth where they are and to provide them with the assistance and support, they need to become self-sufficient or obtain the necessary services they need to exit a life on the streets.
Urban Peak provides numerous programs and services that are founded on the principles of trauma-informed care and Positive Youth Development to assist youth in reaching their potential and living a successful life off the streets. Services include street outreach, a 136-bed shelter for youth ages 12 through 24, daytime drop-in services, supportive housing, education and employment services, case management, life skills courses, behavioral health evaluations, meals and more.
