Position Summary
UVS is seeking a dedicated and organized Service Coordinator to join our team. In this role, you will serve as the primary liaison between participants, families, shared living providers, and state systems, ensuring that services are delivered in accordance with Vermont regulations and aligned with each individual’s personal goals.
This position is ideal for someone who is detail-oriented, confident, highly organized, and passionate about supporting individuals with developmental disabilities.
Key Responsibilities
- Person-Centered Planning & Support
- Facilitate the development, implementation, and ongoing review of individualized service plans in collaboration with participants, families, and care providers
- Ensure all supports align with each participant’s goals, preferences, and needs
- Advocate for participant rights, independence, and inclusion
- Home and Community Visits
- Conduct monthly (or more frequent as needed) visits in participants’ homes
- Monitor health, safety, and overall well-being
- Support shared living providers in maintaining safe, supportive environments
- Care Coordination & Communication
- Act as the central point of contact among participants, providers, families, and community partners
- Coordinate services including healthcare, employment supports, and community engagement opportunities
- Build and maintain strong working relationships with stakeholders
- Documentation & Compliance
- Maintain accurate, timely, and detailed documentation in compliance with state and agency standards
- Track progress toward goals and ensure required reporting deadlines are met
- Monitoring & Quality Assurance
- Evaluate effectiveness of services and recommend adjustments
- Ensure compliance with Vermont state regulations for developmental disability and Adult Family Care services
- Crisis Response & Problem Solving
- Respond to urgent situations with sound judgment and professionalism
- Support providers and families through challenges
- Independent & Team-Based Work
- Manage an independent caseload while collaborating with a supportive team
- Balance fieldwork with office responsibilities in Moretown

- Strong written and verbal communication abilities
- Ability to meet deadlines and manage multiple priorities
- Experience working with individuals with developmental disabilities preferred
- Ability to work independently and as part of a team
- Confident decision-making and problem-solving skills
- Comfortable with documentation and computer systems
Education & Requirements
- Bachelor’s degree in Human Services or a related field or an equivalent combination of education and relevant experience
- Valid driver’s license and reliable, insured vehicle (required)
- Ability to pass required background checks
Work Schedule & Location
- Monday–Friday, 8:00 AM – 4:00 PM
- Occasional evening or weekend availability
- Hybrid role: a mix of fieldwork and office time based in Moretown, VT
