Job Description

General Purpose of Job:

Provides spiritual care to patients, their families and significant others. An active member of the interdisciplinary team, providing spiritual support to hospice caregivers as appropriate.

Essential Duties

This job description is intended to cover the minimum essential duties assigned on a regular basis. Associates may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.

• Initiate spiritual care services when referral is received by the admitting nurse.
• Complete a spiritual assessment of each patient/family system within five days of admission to hospice services, unless spritual care services are declined by the patient/family.
• Provides supportive presence and listening without judgment to hospice patients and loved ones.
• Complete documentation of spiritual assessments in the electronic medical record within 48 hours of making the contact to the patient/family.
• Attend the biweekly Interdisciplinary Team Meetings to review patient care and care plans with IDT members.
• Maintain communication with IDT members related to patient care plan and patient/family needs.
• Assist in the development of spiritual care programs, projects and policies.
• Maintain appropriate, up to date and relevant education related to spiritual care at end of life.
• Follow HIPPA guidelines related to privacy and confidentiality of the patients/families being served. This includes ensuring safe keeping of the electronic medical record and laptop computer provided for documentation.
• Complete all health system required educational competencies in a timely manner.
  
  

Job Requirements

General Requirements

• Active American Heart Association BLS Certification upon hire
• Valid Driver's license
• Current Proof of Auto Insurance
  
  

Work Experience

• Hospice or other healthcare experience preferred.
• Ordained person, as determined and indicated by their denomination, and/or trained and experienced lay person, active member of an area congregation.
• Background in healthcare, hospice and/or palliative care - preferred.
  
  

Education

• GED or High School Diploma with work toward or completion of CPE.
• MDiv, theological degree, or related coursework preferred.
  
  

Specialized Knowledge and Skills

• Transportation and ability to travel in order to make home and facility based visits required.
• Able to differentiate between spirituality and religion in order to meet patients and families where they are at in terms of their own beliefs and incorporate this within a diverse population.
• Excellent assessment, communication and documentation skills with ability to creatively problem solve.
• Able to manage diverse patient caseload, potentially stressful situations, and interpersonal conflict.
• Able to develop and maintain a working relationship with community agencies in area and network effectively.
• Ability to work with an interdisciplinary team.
• Demonstrates the ability to obtain information on the teachings and practices of the religion of those served.
• Possesses strong verbal and written communication skills.
• Demonstrates the ability to successfully interact with the diverse population of hospice.
• Able to utilize time effectively and efficiently, to organize workday and set priorities.
• Able to work independently.
• Strong knowledge of electronic medical records and working with computers - preferred.
