Job Requirements

POSITION SUMMARY:

Under close supervision, the HIM Technician prepares, scans, and verifies all documents scanned in the electronic medical record. The position reviews, compiles, indexes and categorizes documents to ensure accuracy and route to correct patient account and folder within the electronic medical record to include the retrieval and reconciliation of all discharged patients records, accounts, et. al.

The following job description covers the most significant duties performed, but does not exclude other work assignments not mentioned

Primary Responsibilities

  • Preps documents includes but not limited to alphabetizing, putting documents in numerical order and/or date order, removing staples, repair forms as needed and etc.
  • Identifies different document types making sure all documents are barcoded accurately to route to specific folder within EMR.
  • Confirms the patient name, medical record number, and account number are on every page in the chart, front and back.
  • Reviews images to determine the need to delete, add or enhance images for clarity.
  • Completes discharge processing- retrieval of records from all units and verifies receipt of all inpatient and outpatient discharged records; follow up on missing records to ensure they are received in a timely manner according to departmental procedures.
  • Updates required shared drive spreadsheets according to department standards.
  • Prepares paper documents for proper storage and easy retrieval.
  • Participates in training programs to facilitate the proper use and implementation of quality systems and processes.
  • Performs related work as assigned.

Customer Service

  • Greets customers in courteous, friendly, respectful and professional manner at all times, including

maintaining eye contact when appropriate.

  • Follows communication protocols to both internal and external customers, including introducing him/herself with job title and experience, asking open ended questions.
  • Responds promptly and appropriately to customer questions/concerns/complaints and attempts immediate resolution.
  • Keeps customer’s information confidential at all times.
  • Provides assistance and offers help immediately, including referring requests to appropriate staff members as necessary, if unable to complete him/herself. Provide staff member with information necessary to complete the request and introduce staff to customer at the time of hand-off. Ensure the customer that the staff member will provide excellent service.

Communication Etiquette

  • Respectful, courteous and professional in all forms of communication and follows facility’s service communication protocol in all interactions.
  • Refrains from use of personal cell phone in HIM department
  • Makes every effort to answer telephone calls within three rings, introducing himself/herself, department and title (if appropriate). Asks permission before placing the caller on hold or using the speakerphone. If caller is transferred, gives the caller the extension number of the person he or she is being transferred to. Offers further assistance to the caller upon completing the conversation.
  • Returns email and voicemail messages promptly but no later than within one business day (24 hours).
  • Always mindful of voice and language in public.
  • Keeps customer’s information confidential at all times.
  • Excellent customer assistance and relations with all customers, patients/clients and, physicians.

Provides co-workers with a status report for continuity of workflow when planning to be out of the office, off the unit, or away from the department

Self Management

  • Reports to work appropriately groomed and in compliance with the Hospital’s dress code. Wears identification badge at all times at chest level and facing outwards so identification is clearly visible.
  • Complete all assignments within deadlines or negotiates alternative actions and time frames in order to achieve desired outcomes.
  • Completes mandatory annual education and competency requirements.
  • Follows DHS safety, infection control and employee health standards.
  • Demonstrates responsibility for personal growth, development and professional knowledge and competency.
  • Adhere to all DHS and department policies and procedures, including Code of Conduct and professional behavior standards. Does not exceed Hospital guidelines in reference to attendance, punctuality, and use of sick and unplanned absences. Provides notification of absences, lateness and vacation requests according to department guidelines. Respects length of time for lunch and break times.
  • Reviews, signs, and adheres to DHS and/or departmental confidentiality statement.

Work Experience

POSITION REQUIREMENTS:

Licensure/Certification/Registration

  • Prefers Accredited/Registered Health Information Technician or Administrator

(RHIT/RHIA) or other American Health Information Management Association (AHIMA) certifications will be considered.

Education/Knowledge

  • AA degree in Health Information Technology or related field; or in lieu of degree, significant acute care inpatient and or outpatient health information management. Experience will be considered equivalent to 4 or more years of acute care EMR experience.
  • One (1) year experience with preparation of medical records, monitoring of scanned documents assuring accuracy and timely completion of electronic medical records required.
  • Two (2) years’ experience in electronic document imaging required.
  • Knowledge of medical record format and content for all patient record required.

Skills

  • Requires attention to detail, repetitious work, logging records, assessing the accuracy, quality control, and validation processes for the electronic medical record.
  • Good/effective communication skills (oral/writing).
  • Speaks fluent English with clarity and understanding.
  • Competent knowledge and working experience with Medical Terminology.
  • Computer Literate.

Understands and practices confidentiality policies and procedures

Compensation

Pay Range: $17.50-28.48
