Responsibilities

• Provide skilled nursing care to hospice patients in their home environment.
• Assess and monitor patient conditions, managing symptoms effectively to ensure patient comfort.
• Coordinate care with interdisciplinary team members, including physicians, social workers, and chaplains.
• Educate patients and their families about disease processes, symptom management, and end-of-life care options.
• Document patient care accurately and timely according to organizational policies and regulatory requirements.
• Respond promptly to patient and family needs, providing emotional support and guidance.

Requirements

• Current and valid Registered Nurse (RN) license in the state of Michigan.
• Experience in hospice, home health, or palliative care preferred.
• Strong interpersonal and communication skills.
• Ability to work independently and manage patient caseload effectively.
• Compassionate with a patient-focused approach to care.
• BLS/CPR certification required.
