UnityPoint Health – Fort Dodge provides coordinated clinic, hospital and home-based care for patients in Fort Dodge and surrounding communities. As a designated regional referral center Trinity Regional Medical Center offers a wide range of primary and secondary care services. Trinity is accredited by DNV Healthcare Inc. and has achieved Pathway to Excellence designation by the American Nurses Credentialing Center.
General
- Flexible role (full time and part-time options available)
- Established team of 1 Medical Director and 4 Advanced Practitioners
- Established practice serving the community
- Experienced staff, both clinical and non-clinical, who know the patients and the community
- Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination
- Full & part-time roles

Clinic hours: Monday-Friday 8am-5pm
- 4 day work week
- No nights, weekends, or holidays
- Palliative Care services provided in patient's homes, clinic and/or nursing home setting
- Average of 8-10 patients per day; no nights or weekends
- Shared call (telephone only) amongst the providers. Currently 1:17 – Triaged through MyNurse
- Evaluates, interprets, and coordinates information to make decisions and recommendations regarding patient care
- Patient-focused, friendly, and willing to collaborate in a team environment
- Excellent communication (written and verbal), analytical and problem-solving skills
- Nurse Practitioner or Physician Associate
- Experience in post-acute and/or long term care facility preferred
- Palliative Care certification preferred
- Ability to work as a team member
- Hold an unrestricted medical license in the state of Iowa
- Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
