Responsibilities:- Develop and maintain the patient’s treatment plan throughout the patient’s course of treatment with input from the care team
- Keep patient, family, or others informed as to the patient’s progress throughout treatment as the patient desires.
- Complete safety planning
- Complete discharge planning
- May lead psychoeducational groups
- Coordinate patient’s social determinants of care needs
- Practice de-escalation techniques
- Provide patient information and make referrals
- Understand Medicaid and other governmental benefit systems and help patients navigate through the public mental health system.
- Help patients follow through with their health goals.
- Coordinate medical and behavioral healthcare for patients
- Assists patients with activities of daily living
- Conducts assessments including but not limited to suicide screening and mental health assessments, patient rights, obtaining releases of information, advanced directives etc.
- May completes patient rounding in order to maintain a safe environment
- Provides culturally and linguistically appropriate services
- May transport patients
Documentation and Administrative Functions:
- Participate in all required trainings and educational activities
- Participate in department level meetings and huddles
- Be responsive to patients in applicable communication mode
Experience: Previous healthcare & behavioral healthcare experience preferred.
License(s)/Certification(s): Driver’s license if in an outpatient setting. May require class D Driver’s license within 6 months of employment for outpatient setting. May be required to obtain BLS within six months of hire for certain sites.
Other: Use of usual and customary equipment used to perform essential functions of the position.
