What are the responsibilities and job description for the Licensed Practical Nurse position at Unity Hospice WI?

This role offers a unique opportunity to work in our Meng Residence, a home-like hospice setting where you can slow down, build meaningful connections, and provide truly patient-centered care. This setting allows you to focus on quality, presence, and comfort for both patients and their families.
Schedule: Primarily based at the Meng Residence in De Pere, working 32–36 hours per week, 9:00 am to 9:00 pm shifts. There will be occasional 3:00 pm to 11:00 pm shifts in the community to support patient care needs, along with an every third weekend rotation and 2-3 holidays per year.

What You’ll Do
- Provide hands-on, compassionate care to patients in a home-like hospice setting
- Support and follow individualized care plans in collaboration with the RN and care team
- Monitor patient conditions and communicate any changes promptly
- Provide education and support to patients and families
- Assist with clinical tasks, including IV monitoring under RN direction
- Document care accurately and maintain compliance with regulatory standards
- Help create a calm, respectful, and supportive environment

- Current unrestricted Licensed Practical Nurse licensure from the State of Wisconsin
- Valid WI Drivers’ License, reliable transportation, and proof of current automobile insurance coverage
