What You’ll Do
- Build relationships with clients and guide them toward independence
- Develop and track individualized housing and service plans
- Connect clients to critical resources and community services
- Support day-to-day needs to ensure safety, stability, and well-being
- Maintain clear, accurate case documentation (HMIS)
- Collaborate with a supportive, multidisciplinary team
What We’re Looking For
- Bachelor’s degree in Social Work or related field
- Experience in case management or human services preferred
- Passion for working with vulnerable populations
- Strong organizational and communication skills
