Qualifications:JOB SUMMARY
The Director of Social Services provides strategic and operational oversight of Community Health Resource Coordination (CHRC) and the Center of Excellence (COE).
This position ensures the integration of social care, substance use disorder (SUD) treatment, and care coordination within Union’s team based, integrated care model to improve health outcomes, advance health equity, and address social drivers of health (SDOH).
The Director is responsible for regulatory compliance, grant management, quality performance, data reporting, cross-departmental integration, and community partnership leadership. This role ensures services are measurable, sustainable, and aligned with Union’s strategic priorities and whole-person care model.
SPECIFIC JOB DUTIES
Leadership & Supervision
- Supervises and supports Community Health Resource Coordinators (Levels I–III), Social Workers and COE team members either directly or through designated leads/managers.
- Provides strategic direction and operational oversight for:
- Social Services / CHRC programming
- Center of Excellence (COE) care coordination
- Establishes clear performance expectations, monitors outcomes, and ensures accountability for program deliverables.
- Promotes a trauma-informed and patient-centered culture across all programs.
Clinical & Program Integration
- Ensures seamless integration of social services and COE programming within primary care, behavioral health, and care management workflows.
- Collaborates with Medical, Behavioral Health, Quality, and Operations leadership to close care gaps and improve outcomes for patients with complex medical, behavioral, and social needs.
- Develops and implements protocols, workflows, and referral pathways to ensure high reliability and effective care transitions.
Compliance, Reporting & Grants
- Ensures compliance with:
- COE standards and state regulations
- HRSA/FQHC program requirements
- UDS reporting standards
- Grant and funding agency deliverables
- Oversees grant reporting, participates in grant writing, and ensures timely submission of required programmatic and financial reports.
- Monitors program fidelity, documentation standards, and audit readiness.
- Analyzes, validates, and reports program data internally and externally to demonstrate impact and return on investment.
Data, Quality & Performance Improvement
- Leads analysis of SDOH, substance use trends, health equity data, and social barriers impacting patient outcomes.
- Develops dashboards and measurable goals for social services and COE performance.
- Ensures services contribute to improved clinical quality measures, utilization outcomes, and value-based care performance.
- Identifies emerging community needs and leads responsive program development.
Community & System Leadership
- Represents Union as a decision-maker on community councils, collaboratives, and cross-sector coalitions.
- Strengthens partnerships with:
- County agencies
- Behavioral health providers
- Recovery support
- Housing and social service agencies
- Managed care organizations
- Builds and maintains strong relationships with funders to maximize sustainable resources.
- Advances collective impact strategies in Lancaster and Lebanon communities.
Strategic Development
- Develops and executes operational plan goals for social services aligned with organizational goals.
- Evaluates program sustainability, staffing models, and expansion opportunities.
Organizational Responsibilities
- Participates in mandatory leadership meetings, all-staff meetings, and trainings.
- Performs other work-related duties as assigned.
POSITION REQUIREMENTS
- Master’s degree in social work, Public Health, Healthcare Administration, or related field or equivalent education and experience.
- Minimum 2 years of leadership experience in social services, public health, behavioral health, or addiction services
- Experience with Center of Excellence (COE) models preferred
- Strong understanding of trauma-informed care and recovery-oriented systems of care
- Excellent written and verbal communication skills
- Advanced analytical skills with ability to interpret and present program performance data
- Ability to collaborate effectively within interdisciplinary teams
