Working Environment
This position may require working in clients' homes within varying environments—some may be multi-story homes, while others might have pets or specific housekeeping conditions to adapt to.
Flexibility in scheduling may be required, including evenings, weekends, and holidays based on client needs.
Physical tasks involved may include assisting with mobility, lifting clients, and performing basic household chores.

- Interdisciplinary Collaboration
- Personal Care: Assist clients with daily living activities, including bathing, grooming, dressing, toileting, feeding, and mobility. Provide compassionate support while maintaining the client’s dignity.
- Mobility Assistance: Aid clients in transferring to and from beds, wheelchairs, or other assistive devices while following safety protocols.
- Health Monitoring: Assist in monitoring and recording vital signs, changes in clients’ conditions, and report any significant changes to the supervising nurse or medical team
- Documentation: Maintain accurate and up-to-date documentation of client care and progress according to state laws and agency policies.
- Interdisciplinary Collaboration: Work closely with family members, healthcare providers, and other caregivers to ensure continuity of care and support for the client and other duties as assigned
- Adhere to all Federal and State home health laws, regulations, survey readiness preparation, and compliance per Agency policy
- Comply with all clinical department and agency policy and procedures
- High school diploma or equivalent; CNA (Certified Nursing Assistant) certification preferred.
- Previous experience as a home health aide, caregiver, or in a similar role is highly desirable.
- Understanding of basic healthcare practices, hygiene, and safety protocols
- Compassionate nature with a strong desire to assist and care for others.
- Excellent communication and interpersonal skills.
- Ability to handle the physical demands of the job, including lifting and assisting clients.
- Reliable, punctual, and able to work independently or as part of a team.
- Valid driver’s license and access to a reliable vehicle or transportation.
- Current and up to date CPR certification
- Background check and drug screening required.
