Your Role
As a Primary Care Nurse, you’ll play an essential role in supporting our providers and delivering hands-on care that helps patients feel heard, understood, and well cared for. From clinical assessments to patient education, you’ll be part of each step in a person’s health journey—building trust through expertise, empathy, and teamwork.
What You’ll Do
- Partner closely with Primary Care Providers to support care coordination and follow-up for patients and families.
- Assist with treatment procedures and carry out provider orders with precision and care.
- Perform patient assessments and collect data through history-taking and physical evaluations.
- Monitor patient status and respond to clinical needs using sound nursing judgment.
- Provide education and counseling to patients and families about diagnoses, treatments, and care plans.
- Conduct direct patient care, including vitals, medication administration, and monitoring.
- Support specialty care referrals by coordinating appointments and follow-ups.
- Document all interactions and updates accurately in the electronic medical record system (EMR).
- Triage patient concerns to ensure timely, appropriate care.
- Contribute to a positive clinic environment by collaborating with team members and helping wherever needed.
Position Details & Work Environment
- Position Type: Full-time, Non-Exempt
- Schedule: Monday through Friday, 8:00am to 5:00pm
- Work Environment: Clinic-based role requiring frequent walking, standing, bending, and lifting.
