What are the responsibilities and job description for the Service Coordinator position at TriSource?
The Service Coordinator supports individuals and families in achieving greater self-sufficiency through case management, workforce development, and connections to community resources. This role helps clients identify goals, overcome barriers, and access employment, educational, and supportive services that promote long-term stability and independence.
What You'll Do
- Assess client needs and develop individualized service plans.
- Provide guidance, support, and informal counseling to help clients achieve personal and employment goals.
- Assist with resume development, job search strategies, interview preparation, and job readiness training.
- Connect clients with employment opportunities, workforce development programs, educational resources, and community services.
- Develop, coordinate, and facilitate educational, life skills, and workforce development programs.
- Build and maintain partnerships with community organizations and service providers.
- Monitor client progress, program participation, and service outcomes.
- Maintain accurate case files, documentation, and data entry in accordance with program requirements.
- Assist with conflict resolution, mediation, and referrals to appropriate support services.
- Participate in outreach activities, workshops, and special projects as assigned.
What You'll Bring
- High school diploma or GED.
- Minimum of two (2) years of experience in social services, workforce development, community outreach, or related programs.
- Strong communication, organizational, and interpersonal skills.
- Ability to build rapport and work effectively with individuals from diverse backgrounds.
- Valid driver's license and reliable transportation.
- Ability to successfully pass a background check and drug screening, if required.
What We Look For
- Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field.
- Experience with workforce development, housing assistance, or other community-based programs.
- Case management, counseling, or client advocacy experience.
- Familiarity with community resources and referral networks.
Pay: $26.00 - $28.00 per hour
