This position will be to assist our Admissions Director and our Social Services Director in daily task.
Position Summary
The Admissions and Social Services Coordinator is in charge of social services tasks such as admissions and transfers, as well as providing social services functions and support to the patients and residents in the facility. One of his/her their functions includes managing the admission process, including the completion all admission paperwork for incoming patients into the facility.
Essential Functions
The Admissions and Social Services Coordinator arranges medical appointments, transportation and other needs for their patients. In all tasks, the Admissions and Social Services Coordinator follows strict privacy and confidentiality regulations.
The Admissions and Social Services Coordinator communicates with patients/residents, family members and staff members. They also provide education regarding rights and benefits, as well as obligations. He/she develops effective integration strategies for residents, participate in the care plan process, keeps accurate records, and maintains ongoing quality measurement. He/she assists the patient/resident and family with financial-planning alternatives and Medicaid applications. He/she identifies residents' psycho-social needs and orients them regarding their options.

Key Responsibilities
- Coordinate the admission/transfer/discharge of patients in accordance with federal, state and local regulations, established procedural guidelines and as directed.
- Obtain pertinent information from patients and/or family members, referral sources, and appropriate social agencies and verify financial information prior to admission and completes admission paperwork with family and patient.
- Sustain contact and provide support to patient/families.
- Orient the patient to the center environment.
- Communicate admissions information to other departments.
- Maintain knowledge and understanding of Medicare, Medicaid, Private Pay and Third-Party reimbursement and provides financial assistance information to patients and family members.
- Assists patients in obtaining needed services when such services are not provided by the center.
- Responsible for meeting documentation requirements for long-term care guidelines related to social services.
- Have a Bachelors degree in Social Work, a related field, or nursing.
- Be customer-service oriented and possess strong marketing and communication skills. Computer skills preferred.
- Have long-term care experience. Prior admission experience preferred but not required.
- Experience in marketing for skilled nursing centers is a plus.
- Possess ability to communicate effectively, both orally and in writing.
- Be capable of maintaining effective working relationships with residents, family members, department heads, and fellow staff.
- Be an excellent problem solver and be able to multitask in a variety of projects by prioritizing crucial tasks.
