Tridant Solutions is seeking qualified, dedicated Non-Clinical RN Case Managers to support Service Members (SM) of the Maine National Guard.

The Non-Clinical Case Manager RN supports service members by coordinating access to essential resources and services. Their duties include assessing individual needs, developing action plans, and connecting soldiers and their families with support programs such as housing, education, employment, financial assistance, and behavioral health referrals. They maintain detailed case records, ensure follow-up, and collaborate with internal and external agencies to promote readiness and well-being—without providing direct clinical care or therapy.

DUTIES/RESPONSIBILITIES:

· Review all electronic data DOD systems i.e., MEDPROS, eCASE, medical readiness portals and behavioral health portals prior to Medical Readiness Events (MRE) to provide early identification of non-deployable soldiers

· Review all open cases every 21 calendar days to ensure all of SM with an open case is being tracked, managed, and communicated

· Interview the soldiers either telephonically or face to face and shall chart this communication into eCase module

· Perform chart, track, and document Service Members interview and interactions in the Medical Data (for) Care History and Readiness Tracking (MEDCHART) applications.

· Will be compliant with maintain all Health Information Portability and Accountability Act (HIPPA) standards.

· Follow the regulations and policies applicable to Individual Medical Readiness (IMR) and deployment

· Scan and index all medical documents received while performing their tasks.

· Attend and perform their required services at all Medical Readiness Events (MRE ) and Soldier Readiness Processing (SRP) events

· Track medical status of all assigned Soldiers

· Schedule profile exams/evaluations and audiology exams at periodic health exam facility.

· Coordinate Soldier treatment plans with appropriate medical facilities.

· Check daily the generated automatic referrals list from MEDCHART DOD systems after every MRE. Schedule a follow-up appointment with an army medical provider to discuss the referral and the need for further actions.

· Correspond with soldiers to obtain information for the Soldier’s case management file, referred by a provider are followed

· Manage Soldier’s care from the identification of the condition to when the Soldier is either considered medically ready and returned to duty or administratively separated from the military.

· Educate Soldiers on resources available to improve medical and dental readiness.

· Review and track health assessment lab results and contact SM and educate on lifestyle modifications.

· Update MODS as required on the following: immunizations, designation of pregnancy, eye exams, and medical warning tags.

· Track medical appointments by communication with unit to ensure Soldier attendance and submission of follow-up documentation.

· Post MRE, run and provide the MRC3/MRC4 reports to National Guard stakeholders NLT 5 days post event.

· Monitor MEDPROS to ensure all scheduled services are completed/updated and current medical status is reflected.

· Provide liaison services and coordinate with the National Guard stakeholders on the most difficult case files.

· Ensure Soldiers with P3 or P4 profiles are brought to the attention of the National Guard stakeholders to ensure referrals shall be tracked.

· Mail all medical records to ensure timely and accurate processing of medical records to and from mobilization sites, 30 calendar days prior to soldiers’ report date.

· Assists with all aspects of Case Management operations and procedures.

· Prepare medical documentation for Line of Duty (LOD) investigation for units for which they are responsible in conjunction with Health Service Support (HSS).

· Enter individual vouchers or individual events for SM IMR requirements into the QTC Reserved health readiness program when a soldier misses a MRE or SRP.

· In coordination with the HSS, lead soldier through the medical board process

· Other duties as assigned

REQUIREMENTS:

MUST possess an unencumbered, active Maine RN license.

MUST possess a nursing degree from an accredited university.

MUST have at least 2 years of case management experience and a minimum of 3 years’ clinical experience.

· Bachelor of Science in Nursing (BSN) is preferred; associate degrees may be accepted with relevant experience

· Experience with Army systems such as MEDPROS, eProfile, or MODS

· Experience in case management or care coordination, ideally in military, government, or healthcare settings.

· Familiarity with Individual Medical Readiness (IMR) tracking and documentation systems

· Strong skills in documentation, communication, and interdisciplinary collaboration.

· Preferred Certified Case Manager (CCM) Certification.

· Preferred minimum of two (2) years’ experience with knowledge of military culture and experience collaborating with social service agencies to integrate resources, plan, and deliver comprehensive services.

Weekend work is required.

Typical weekend events occur from November – April or annual PHA time. During that time, one weekend a month will be required; the rest of the time, weekends will not be required.

Occasional in-state reimbursable travel is required.

COMPENSATION

$36.48-$42.83/per hour DOE
