What are the responsibilities and job description for the LPN (Licensed Practical Nurse) position at Tri-County Family Medicine?
We are seeking a motivated professional to join our team in a full-time role. This position offers a family-focused work environment with no overnights or holidays and only occasional Saturdays.
Key Responsibilities:
- Interpret and carry out provider orders while maintaining high standards of patient care
- Coordinate patient services within the hospital and community; document all care accurately in the electronic medical record (EMR)
- Demonstrate a compassionate and culturally sensitive approach to patients and their families
- Assist with patient and family education to support understanding of care plans and treatments
- Obtain and accurately record patient histories and vital signs
- Review and update medication lists at each visit; prepare prescription refill requests for provider approval and electronic transmission
- Support efficient communication between patients, families, hospitals, and providers (including telephone coordination)
- Complete daily and monthly nursing checklists and logs; promptly report abnormalities to the Nursing Supervisor
- Contribute to a team-oriented environment in a community health setting serving diverse populations regardless of ability to pay
- Bilingual skills are a plus, to support diverse patient populations
