What are the responsibilities and job description for the Information Management Document Clerk position at Tri-County Family Medicine?

- Electronically scan all signed and unsigned incoming lab, radiology, procedure reports, consult notes and any other related correspondence into the appropriate folder of the patient’s chart and then assign it to the appropriate provider.
- Assist all staff if necessary with locating any document as needed.
- Faxing of documents to the appropriate destination.
- Entering of RHIO consents and declinations into the RHIO system then scanning these documents into the appropriate folder in the patient’s chart.
- Entering UDS information into the appropriate sections of the patient chart then scanning these documents into the appropriate folders of the patient’s chart.
- Monitors failed faxes for all offices. Resends failed faxes and/or locates a correct fax number for resending.
- Corrects documents found to be in the incorrect chart. Places documents in the correct patient chart in the appropriate folder and then deletes the document found in the incorrect chart.
- Utilizes computer systems for retrieval of patient information relative to work assigned.
- Performs other duties as required.
- Hours may vary.
