Options Counselor for Coos County TCCAP ADRC (formerly known as ServiceLink)
The TCCAP ADRC (Aging and Disability Resources Center) is looking for the right candidate to join our team to help support the aging and disabled population of Coos County.
This position provides a single point of entry into the long-term supports and services system for older adults and people with disabilities of all income levels. The right candidate for this position will specialize in assisting people who are aging or disabled with getting the support and resources they need to maintain their highest level of independence.
This is a person-centered role who conducts comprehensive needs assessments , clinical eligibility counseling, long- term support counseling, options counseling, program pre-screening eligibility, assists with Medicare enrollment at Open Enrollment, Medicaid assistance, and client follow-up.
This position participates in continuing education and peer counseling workshops and activities. Assists with/provides periodic public education sessions and engages in inter-professional and inter- agency collaboration and education regarding long-term support counseling services.
Examples of Key Functions of ADRC Staff
- Options Counseling: Guiding individuals through long-term care decisions.
- Information and Referral: Connecting callers to services like home care, meals, and transportation.
- Benefit Support: Assisting with Medicare, Medicaid, and Social Security applications.
- Care Transitions: Supporting individuals moving from hospitals or nursing homes back to the community
A bachelor's degree in human services or a related field is preferred but experience may be considered to the right candidate.
SHIP certification or willingness to obtain is required.
Job Type: Full-time
