CASE MANAGER – Conflict Free  

 

Senior and Disabled Adults Services

 

Full-time (40 Hours per week) Location: Johnston

 

General Responsibility:

Provide Case Management activities to the Elder Adults Disabled (EAD) population on Medicaid Home Community Based Services (HCBS), for a variety of Long-Term Services Supports setting options including; Self-Directed, Assisted Living, Homecare.  Overall responsibility for the major components of the Conflict Free Case Management service system, case management process, including information gathering, person centered plan development, connections to services and supports, plan monitoring and follow up and related case management activities.

 

Specific Responsibilities: 

Utilize person centered planning approaches to access services the participant needs and wants. Take the HCBS participant’s history to identify the needs of the HCBS participant by gathering information directly from the HCBS participant and other sources, such as family members, HCBS providers, medical providers, social workers, and educators. obtain in-depth information about the participant’s current strengths, resources (including formal and informal support systems), problems, needs, and quality-of-life goals. Assist the HCBS participant to identify person-centered goals based on identified wants and assessed needs. Develop a person-centered plan that is comprehensive and reasonably assures the health and welfare of the participant, acknowledges the participants strengths, promotes the participants self-determined goals, addresses all the participant assessed needs, include to mitigate all identified risks, and accommodate participant preferences to the extent possible within the established service limitations and availability of local resources. The case Manager is expected to at minimum, monitor the participants wellbeing monthly, however monitoring activities and contacts may occur with the participant, family members, service providers, or other entities or individuals as frequently as necessary. Conduct twice yearly face to face visit with the Participant and/or individual representative. Review and update the person-centered plan at least annually or more frequently upon request by the participant or in response to a “significant change” in the participant’s health, functional capacity, social or physical environment, formal or informal support system, or if other circumstances require re-evaluation of the person-centered plan. Report mistreatment, abuse, neglect, exploitation or a harmful act towards a participant, to the appropriate protective services entity based on the participants residence and age group. 


Qualifications:

Preferably, possess a bachelor’s degree, from an accredited college or university, preferably in counseling, Education, Gerontology, Human Services, Nursing, Rehabilitation, Social Work, Psychology, Social Services, Behavioral Health Science, or related field. Or a combination of post-secondary college, professional certifications, certificates, or licenses, in a related field.  Two years of relevant experience (e.g. providing case management or other type of assistance) working with the target population, low income elders and persons with disabilities.  Possess and maintain a valid driver’s license, automobile insurance, and have access to reliable transportation. Be willing to travel to statewide. Possess knowledge of human behavior and aging process. Demonstrate the ability to work with people from various cultures and lifestyles in the helping process. 


Special Qualifications:

Pass initial and ongoing state and national criminal background check. Bi-Lingual with English and Spanish speaking abilities preferred but not required. Possess strong computer knowledge and be skilled in Microsoft Office products, and other computer system databases. Possess knowledge of community resources, insurance, and the provision of health care delivery systems Possess strong organizational, verbal and written skills. Ability to coordinate and network with a wide variety of agencies and professionals involved in providing services to older persons and adults with disabilities. Demonstrate an understanding of everyone’s experiences, every day. Ensuring the delivery of services that recognizes and respects that every moment matters.
