Job Summary
As a CFCM Case Manager, you will carry a caseload of individuals enrolled in Rhode Island’s Developmental Disabilities (DD) Waiver program, providing independent, person-centered support coordination — free from any conflict of interest with direct service providers. You will develop and monitor Individual Support Plans (ISPs), connect participants to services and community resources, and serve as a trusted advocate.
Duties
- Develop and maintain person-centered plans (PCPs/ISPs) in WellSky for assigned participants
- Conduct required face-to-face visits and annual planning meetings
- Coordinate services with residential, day, and employment providers
- Monitor service delivery, participant health/safety, and goal progress
- Document all case activity in WellSky in accordance with BHDDH timelines
- Assist participants in understanding and exercising their rights and self-direction options
- Participate in agency meetings, training, and quality assurance activities
Qualifications
- Bachelor’s degree in social work, psychology, human services, or a related field — or an equivalent combination of education and at least two (2) years of direct experience supporting individuals with intellectual or developmental disabilities
- Knowledge of Rhode Island’s BHDDH DD Waiver, HCBS standards, and person-centered planning principles
- Strong documentation skills; experience with electronic case management systems preferred
- Valid Rhode Island driver’s license and reliable transportation (community-based position)
- Ability to pass a criminal background check
Pay: $55,000.00 - $65,000.00 per year
