Position Summary
The Licensed Vocational Nurse (LVN) provides high-quality, individualized nursing care to residents in a Skilled Nursing Facility (SNF). Working under the direction of nursing leadership, the LVN collaborates with physicians, nursing staff, residents, families, and interdisciplinary teams to ensure safe, compassionate, and effective patient care, with a strong focus on geriatric and rehabilitation services.
Qualifications
- Active California LVN license
- Minimum 2 years of SNF or long-term care experience
- Current BLS/BCLS certification (ACLS/PALS/NRP preferred if applicable)
- Strong interest in geriatric and rehabilitation nursing
- Ability to work effectively in a fast-paced, resident-centered environment
- Excellent physical stamina with ability to walk, bend, lift, and perform required nursing tasks
- Strong assessment, communication, and critical-thinking skills
- Dependable, adaptable, and team-oriented with a professional demeanor
- Ability to communicate clearly in English, both verbally and in writing
- Provide direct nursing care in accordance with established policies and procedures
- Perform charge nurse duties using sound nursing principles
- Administer medications safely and accurately, ensuring proper documentation
- Conduct resident assessments, including new admissions, changes in condition, falls, or injuries
- Communicate resident condition changes to physicians and implement orders promptly
- Assist with care planning, resident education, and discharge planning
- Supervise and assign nursing assistants to ensure quality resident care
- Respond appropriately to emergency situations and document interventions
- Maintain accurate and timely clinical documentation
- Promote safety, infection control, and regulatory compliance
- Support orientation and education of new staff as needed
- Maintain a professional appearance and adhere to dress code policies
- Uphold confidentiality and ethical standards at all times
- Demonstrate strong teamwork and positive working relationships
- Attend required meetings, in-services, and continuing education programs
- Maintain compliance with all regulatory and licensing requirements
- Shifts may include days, nights, weekends, and holidays
- Shift lengths may be 8, 10, or 12 hours, based on department needs
- Ability to work beyond normal hours or float to other roles if required
- Must be able to lift up to 35 pounds and remain physically active throughout shifts
