Transitions LifeCare Hospice is currently hiring a Clinical Director (RN)

Job Summary We are looking for an experienced Registered Nurse with experience in Hospice with prior management or lead experience. The Director of Clinical Services is a registered professional nurse who ensures quality of patient care in the patients residence. Primary function is for the overall administration of the clinical departments and monitoring of appropriate staffing and productivity. Coordinates care with the interdisciplinary team, patient/family and referring agency. Works closely with our QAPI RN and directs clinical staff with regard to agency policies and procedures.
Responsibilities:
-Directs and coordinates clinical departments and sites, assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and Federal regulations. (Conditions of Participation)
-Promotes the philosophy and administrative policies of the Agency, in an effort to ensure quality of patient care.
- Supervising and coordinating patient care in an effort to ensure quality, complical with Plan of Care, assessment and reassessment of patient's needs and continuity of services by appropriate health care for the specified team of Registered Nurse Case Managers, RNs, Licensed Practical Nurses and Hospice Aides.
- Collaborate with other departments to ensure effective coordination of patient care
- Ensures compliance with Medicare Conditions of Participation, state regulations and nurse practice act
-Participates in planning and implementation of staff education programs for all team members
- Participate in strategic planning and decision-making processes using effective interpersonal relations and communication skills. Facilitates the use of these skills with other team members to achieve desirable outcomes.
- Participates in QAPI Program and assures appropriate corrective measures are performed
- Director is a member of the Interdisciplinary Team (IDT) and assist in development of the IDT plan and participates and assures appropriate QAPI corrective measures are performed.
-Must be available at all times either in person or via telecommunications.
- Performs other related duties and responsibilities as assigned
Qualifications
-State licensed as a Registered Nurse, current Drivers License
-Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
-Nursing skills as defined as generally accepted standards of practice. excellant interpersonal skills and ability to communicate effectively. Demonstrates proven decision making skills.
Experience
- Minimum of 2 years Hospice/Home Health experience
- Experience in a healthcare supervisory/management role
- Knowledge of Hospice Federal, and State Regulations
- Excellent leadership and communication skills
- Proven ability to effectively manage a team of clinical professionals
- Experience in building solid, cohesive team
