Job Summary
This is a full-time professional position. Position provides administrative and clinical operations management of Transitions Palliative Care program, special programs (such as GUIDE Model, Empassion partnership, BCBS contract, etc), and growth of upcoming serious illness endeavors. Manages daily operations of the assigned program/area including all administrative, fiscal, and technical activities to ensure high quality health care services. Complies with federal, state, and local regulatory requirements to maintain high level of quality standards. Acts as a resource and facilitates collaboration between team members and other healthcare disciplines throughout the organization and within the healthcare community. The schedule for this position is Monday-Friday 8:30am-5:00pm; weekends and holidays as scheduled.

Administrative Responsibilities (70% of time)
- This position performs all supervisory responsibilities for the Palliative Care Team Lead and other roles as program grows.
- In collaboration with the Chief Medical Officer, leads, develops and coordinates all serious illness and palliative care department initiatives and programs to ensure delivery of high-quality care.
- Meets regularly with serious illness/palliative care staff to review departmental policies, procedures, and protocols, review productivity, to educate on current trends and clinical knowledge, and to discuss operational challenges and opportunities.
- In collaboration with CMO and clinical leadership, sets clear program goals and objectives and develops quantifiable metrics in alignment with the agency’s annual strategic plan.
- Assists in the development of program dashboard for use in oversight of the program
- Continuously evaluates the effectiveness of program strategies and adapts them based on new opportunities in palliative care delivery and reimbursement.
- Seeks out and builds strong relationships with community partners and referral sources to grow the agency’s program. Participates in contract negotiation, contract implementation, and contract review as appropriate.
- In cooperation with the CMO and clinical leadership, develops, recommends, and implements required program policies and procedures.
- Works closely with CMO and CFO to develop departmental budget, tracks progress to budget monthly and manages expenses within established limits.
- Ensures compliance with the Center for Medicare Services (CMS), Medicaid and any other financial institutions/organizations in relation to claim submissions, and payment for all care visits.
- Meets regularly with Chief Medical Officer to review progress toward goals, identify program challenges and opportunities, and position program for success.
- Collaborates with other program directors to ensure the seamless transition of patients to different levels of care offered by the organization.
- Establishes and maintains effective open channels of communication within and across departments that foster accountability, innovation, teamwork, and quality to achieve organizational goals.
- Works with the education department, APP clinical manager, CMO, and clinical leadership to ensure that staff have appropriate orientation and continuing education about best practices in palliative care, primary care, and serious illness management.
- Participates in QAPI activities and other committees as assigned.
- Maintain expertise and keep current with in palliative care, primary care, and serious illness management healthcare trends and practices.
- Participate in professional organizations to advance the profession and improve palliative care, primary care, and serious illness management health care delivery to patients.
- Participate in leadership development, staff development and staff support activities.
- Completes performance evaluations for all assigned staff in a timely manner.
- Participate in research to improve palliative care, primary care, and serious illness management.
- Perform other duties as assigned by supervisor.
- Provide Palliative Care Consultation, Hospice visits and primary care to patients in residential facilities, home settings, Hospice Inpatient Facility and hospitals
- Provide direct patient care on an ongoing basis as directed by the Chief Medical Officer
- Attend IDG meetings as appropriate
- Document in patient medical record to meet federal, state, regulatory, and agency guidelines
- Work with the Chief Medical Officer and Hospice/Palliative Care Physicians to educate hospital staff, residential facility staff, and the community physicians about palliative care pain, and symptom management, communication around end-of-life decision making and other programs available through Transitions LifeCare
- Participate in on-call rotation as assigned
- Perform other duties as assigned by supervisor.

- Master’s degree in the appropriate clinical field
- Strong leadership skills and at least 2 years of leadership experience in a healthcare environment
- Current knowledge of palliative care, hospice, primary care, Accountable Care Organizations (ACOs)
- Demonstrated performance improvement methods and experience
- Critical thinking
- Knowledge of best practices, evidence-based care, centers of excellence, and emerging healthcare trends
- Analytical and organizational assessment skills
- Program planning and development experience
- Strong communication and presentation skills with ability to translate complex ideas into simple concepts
- Proven community networking and collaboration skills
- Knowledge of change management practices
- Experience with the accreditation process
- Excellent computer skills preferred.
- Must have access to transportation.
- Must be able to travel to, enter, and function in the workplace.
- May be required to lift and/or carry items up to 50 pounds.
- Must have sensory abilities to complete work assignments, communicate with internal and external customers and to complete the reporting and follow-up documentation responsibilities of the job.
