The schedule for this position is Monday-Friday 8:30am-5:00pm; holiday rotation.

Transitions LifeCare is seeking a compassionate and skilled Palliative Care Social Worker to join our interdisciplinary team. This full-time role provides emotional, psychosocial, and resource support to patients and families coping with life-limiting illness. The ideal candidate will bring clinical expertise, a deep understanding of palliative care philosophy, and a commitment to delivering person-centered care in home, hospital, and facility settings. Key Responsibilities
- Conduct psychosocial assessments to identify emotional, social, and resource-related needs.
- Facilitate goals-of-care conversations and provide counseling related to illness and treatment.
- Educate patients and families on palliative care, hospice, and related services.
- Collaborate with the palliative care team to develop and implement patient-centered care plans.
- Document all assessments, interventions, and outcomes in accordance with agency and regulatory standards.
- Provide bereavement support and culturally sensitive care tailored to individual needs.
- Maintain strong knowledge of community resources and facilitate appropriate referrals.
- Participate in staff meetings, training, and performance improvement initiatives.
- Education: BSW or MSW from an accredited university
- Experience: Minimum of one year in a healthcare setting (including internships)
- Strong clinical assessment and communication skills
- Knowledge of Medicare, licensing regulations, and social work ethics
- Proficient with technology and electronic documentation
- Valid driver’s license, reliable transportation, and ability to travel throughout the service area
- Ability to lift up to 50 lbs and meet sensory/physical requirements for patient care
