OUTREACH CASE MANAGER

The Town of Westborough seeks applicants for the full-time (40 hours/week) position of
Outreach Case Manager at the Westborough Senior Center. Under the supervision of the Town’s
Senior and Social Services Director, this position serves as an advocate for older adults in the
community and is responsible for providing professional counsel, information, referrals and
follow-up to elders and their families on a variety of issues.
Qualifications: Master of Social Work (LICSW preferred); three to five years of progressively
responsible professional experience in human services, or equivalent combination of education
and experience. Position requires a valid driver’s license, and candidates must be available to
work some evenings and occasional weekend hours.
The ideal candidate will be an energetic individual with the demonstrated ability to work
independently and to assess an individual’s service needs with care and empathy; must maintain
a high degree of confidentiality regarding information obtained from clients. Requires the ability
to engage with people of diverse cultures and backgrounds in a sensitive, respectful and
affirming manner and to foster collaboration with other Town Departments, community groups
and agencies to support the agency’s mission. This position is a mandated reporter.
Pay range: $29.14 to $41.21 per hour based on demonstrated qualifications and experience.

SUMMARY: Provides professional and direct social services work to the community’s seniors
and their families dealing with issues of aging, housing, care giving, nursing home placement and
inter-familial conflict resolution by performing the following duties personally or through
subordinates.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may

Assist seniors in defining their needs, refers clients to appropriate services and facilitates access
to services. Provides information regarding the resources available including housing, health
care, home health care, homemakers, transportation, fuel assistance, nutrition, and a variety of
legal and financial matters.
Meet with family and/or other caregivers to address physical limitations, personal resources,
housing issues and support needed to improve seniors’ quality of life; identify needs, initiate
crisis intervention and support services for seniors where health, safety, social or emotional
issues require assistance.
Provide guidance and assistance to seniors and/or family members in the preparation of forms
and documents necessary to obtain local, state, federal or other benefits and services.
Assist residents on a confidential basis with information, referral and advocacy in person, over
the phone, via email and through home visits.
Promote seniors’ participation in activities and education programs to promote well-being.
Develop and maintain information and referral networks with various community, health and
social service agencies and Town departments. Collaborate with town, local, regional, and other
organizations to optimize client service delivery and resource and referral information.
Maintain records regarding client and related contacts. Record information in compliance with
Executive Office of Elder Affairs guidelines and/or industry best practices.
Mandated reporter of suspected elder abuse, self-neglect.
Provide advocacy and support to residents, their families, and caregivers. Educate the public
regarding programs and services available.

Provide crisis intervention assistance and referrals to social service agencies and resources.
Perform outreach to clients via telephone calls, home visits, email, messenger, and responding to
phone and in-person inquiries at the senior center.
May represent the Senior Center on local committees (e.g., Human Services Alliance, Hoarding
Task Force, Westborough CARES, etc.)
Assists Program and Volunteer Coordinator and Director in developing programs to address
emerging needs of the aging population. May conduct programs to meet the health and welfare
needs of elders.
Coordinates annual Holiday Giving Program.
Tracks outreach activities and prepares and presents reports to the Council on Aging and the
Director when required.
Attend and participate in professional group meetings; stay abreast of new trends and
innovations in the field of senior services.
May assume duties of the Director and/or act on Director’s behalf in his/her absence.
Performs all other related job duties as necessary.
Supervisory Responsibilities: Directly supervises two subordinate staff. Carries out supervisory
responsibilities in accordance with the organization's policies and applicable laws.
Responsibilities include planning, assigning, and directing work.
Education and/or Experience: Master of Social Work required, LICSW preferred; three to five
years of progressively responsible professional experience in human services, or equivalent
combination of education and experience.
Language Skills: Ability to read, analyze, and interpret general business periodicals,
professional journals, technical procedures, or governmental regulations. Ability to write
reports, business correspondence, and procedure manuals. Ability to effectively present
information and respond to questions from groups of managers, clients, customers, and the
general public.
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using
whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and
to draw and interpret bar graphs.
Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid
conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or
diagram form and deal with several abstract and concrete variables.
Computer Skills: To perform this job successfully, an individual should have knowledge of
Internet software; Spreadsheet software and Word Processing software.
Certificates, Licenses, Registrations: Must have a valid driver's license.
Other Skills and Abilities: Ability to deal appropriately with employees, other departments, and
the general public, and in particular, the elderly. Ability to perform detailed work accurately and
efficiently within deadlines. Ability to prioritize multiple tasks and deal effectively with
interruptions. Knowledge of elder network, programs and services. Ability to maintain sensitive
and confidential information.
Other Qualifications: Must pass CORI check. Must be able to meet with clients using personal
means of transportation. May occasionally require flexibility to attend meetings outside of
normal business hours.
