Role Description
This is a full-time hybrid role for a Case Manager based in Los Angeles, CA, with flexibility for some remote work. The Case Manager will be responsible for overseeing client care, coordinating treatment plans, and ensuring access to necessary resources. Responsibilities include conducting client assessments, developing and monitoring individualized care plans, working collaboratively with therapists and other team members, and providing education, advocacy, and support to clients and their families to facilitate sustainable recovery and mental health progress.
Qualifications
- Case management experience, including creating and overseeing individualized treatment plans
- Strong interpersonal, communication, and counseling skills to build trust and rapport with clients and their families
- Knowledge of mental health and substance use treatment modalities, including EMDR, group therapy, and other therapeutic approaches
- Problem-solving and organizational skills, with the ability to manage multiple clients and prioritize tasks effectively
- Proficiency in using electronic health record systems and basic computer applications for documentation and communication
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field (Master's degree preferred)
- Experience in a behavioral health, addiction recovery, or social services setting is a plus
- Ability to work well both independently and within a multidisciplinary team
