Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team at the Torrington Center for Nursing and Rehabilitation, providing exceptional care to our short-term rehabilitation and nursing care patients in Torrington, Connecticut. As an LPN, you will be responsible for delivering high-quality patient care under the supervision of our experienced nursing staff.
Responsibilities:
Provide direct patient care as assigned, including vital sign checks, medication administration, and wound care
Assist patients with activities of daily living (ADLs), such as bathing, dressing, and grooming
Monitor patient's conditions and report any changes or concerns to the registered nurse or physician
Collaborate with the interdisciplinary team to develop and implement patient care plans
Maintain accurate and detailed records of patient care and treatment
Participate in quality improvement initiatives to enhance patient outcomes
Requirements:
Current LPN license in the state of Connecticut
High school diploma or equivalent required
