Job Summary:

Monitor and report changes in patients' conditions to the Nursing Supervisor
Maintain accurate and detailed records of patient care and treatment
Participate in patient education and discharge planning
Collaborate with the interdisciplinary team to ensure comprehensive and individualized care
Requirements:
Current LPN licensure in the State of Connecticut

Ability to lift, transfer, and reposition patients as needed
Strong communication and interpersonal skills
