We are seeking a dedicated and highly skilled Home Health Registered Nurse to provide comprehensive nursing care in patients' homes. Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individual healthcare needs.
- Administer medications via various routes such as injections, IV infusions, and catheterizations while adhering to aseptic techniques and safety protocols.
- Provide wound care, including suturing and ostomy management, ensuring optimal healing and infection prevention.
- Manage complex medical conditions such as chronic illnesses, acute pain episodes, infectious diseases, and trauma cases with precision and professionalism.
- Utilize electronic health record (EHR) systems like Epic, Cerner, Athenahealth, or eClinicalWorks for accurate documentation of patient encounters and medical records.
- Educate patients and their families on disease management, medication administration, nutrition, and health coaching tailored to individual needs.
- Coordinate care with multidisciplinary teams including physicians specializing in ophthalmology, geriatrics, pediatrics, behavioral health, or occupational health as required.
- Perform specialized procedures such as laboratory specimen collection/processing, phlebotomy, tube feeding management, ventilator support, and dialysis treatments.
- Engage in triage activities to prioritize urgent cases effectively while providing compassionate support during emergency or urgent care situations.
- Ensure compliance with HIPAA regulations and maintain confidentiality of all patient information at all times.
- Participate in discharge planning and case management activities to facilitate seamless transitions across care settings such as hospitals or nursing homes.
- Maintain proficiency in ICD-9/ICD-10 coding for accurate documentation and billing purposes.
- Demonstrate a strong understanding of infection control practices and adhere to OSHA standards to ensure a safe environment for patients and staff alike.
Requirements
- Valid RN license with current registration in the state of practice; additional certifications such as BLS (Basic Life Support) are required.
- Proven experience in home health nursing with familiarity in medical-surgical units or hospital medicine environments preferred.
- Extensive clinical expertise across diverse specialties including critical care (ICU), emergency medicine (ER), trauma medicine (Level I & II trauma centers), hospice & palliative medicine, geriatrics (including memory care), pediatric nursing (including neonatal ventilator management), and behavioral health.
- Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth, or similar platforms for documentation purposes.
- Strong knowledge of physiology, anatomy, pharmacology related to medication administration and patient assessment techniques.
- Experience working with people with disabilities or developmental disabilities is highly valued; familiarity with long-term care settings like assisted living facilities or nursing homes is advantageous.
- Ability to perform complex procedures including suturing, catheterization (urinary or vascular), IV insertion/infusion therapy, sterile processing techniques, ostomy care, gastrostomy tube management, and spinal taps if applicable.
- Demonstrated competence in infection control protocols and aseptic techniques during invasive procedures.
- Excellent communication skills for effective patient education and interdisciplinary collaboration within diverse healthcare teams.
- Flexibility to travel within the community or between facilities; valid driver’s license preferred but not mandatory depending on location requirements.
