Our Community Living Department is seeking a dedicated and dynamic Case Manager for our Supported Living Program, serving the San Francisco, San Mateo, and East Bay Areas. About the Role

As a Program Case Manager, you will be at the heart of our mission, overseeing 6-8 Supported Living cases and ensuring that our clients lead fulfilling lives in the communities of their choice. Your role will be pivotal in providing support, quality assurance, and effective management to enhance the independence and well-being of those we serve.

Key Responsibilities

  • Emergency Response: Manage and respond to client and staffing-related emergencies, ensuring safety and continuity of care.
  • Client Engagement: Maintain regular, meaningful contact with clients and their support networks, fostering independence and community integration.
  • Staff Leadership: Recruit, train, and mentor your team, ensuring they are equipped and inspired to provide exceptional support.
  • Administrative Excellence: Oversee scheduling, training compliance, and safety procedures, ensuring smooth operations and the highest quality of service.
  • Community Representation: Serve as a compassionate ambassador for Toolworks, upholding our values and fostering a positive community presence.

What We're Looking For

  • Education: A Bachelor's degree in Special Education, Rehabilitation Counseling, or a related field is preferred but not required.
  • Experience: At least three years in a role supporting adults with disabilities, with a preference for those with case management experience. Supervisory experience is also valued.
  • Skills: Exceptional report writing, strong time management, and a knack for recognizing and resolving challenges.
  • Requirements: A clean DMV printout, First Aid/CPR certification, and the ability to travel within the designated counties are all essential.
