Information & Referral Specialist - Community Outreach (Disability & Senior Services)
Location: San Francisco, CA | Schedule: Full-time, In-Person | Pay: $28.00 - $32.00 p/h DOE

Toolworks is seeking a dedicated and compassionate Information & Referral Specialist to support our Aging and Disability Resource Center team. This is an opportunity to help people with disabilities and older adults access the services and supports they need to live independently and participate fully in their communities.

In this full-time, in-person role, you will be based at our San Francisco headquarters and travel regularly throughout the Bay Area for community outreach, events, and client meetings. You will provide information, direct referrals, resource navigation, and follow-up support to people seeking services such as adult day programs, housing assistance, financial support, transportation, legal assistance, and health care resources.

Professional Development Opportunity: Candidates with a Master of Social Work degree and an active California Associate Clinical Social Worker registration are encouraged to apply. BBS-qualified clinical supervision may be available to eligible employees as part of Toolworks' professional development program. The amount and category of experience eligible for LCSW licensure credit will depend on the employee's assigned duties, the program setting, the supervision plan, and current California Board of Behavioral Sciences requirements. Not all hours worked in this position will qualify toward LCSW licensure.

Your days will be varied and meaningful. You may begin the morning helping someone complete a housing application, spend the afternoon conducting outreach at a senior center, and finish the day documenting referrals and follow-up activities. You will also welcome visitors, assist with front-desk operations, and provide administrative and communication support as part of an inclusive and responsive team.

We are looking for someone with at least two years of experience working with adults with disabilities, older adults, or other underserved communities, preferably in a community-based setting. Strong organizational, interpersonal, and written communication skills are essential. Bilingual abilities in American Sign Language, Spanish, or Cantonese are highly desirable.
