Three Rivers Hospice is seeking a compassionate and motivated Admissions RN to join our growing team! This position offers a unique 7 on / 7 off schedule, providing excellent work-life balance while allowing you to make a meaningful impact in the lives of patients and families.
Schedule
- Week 1: ON
- Monday–Friday: 2:00 PM – Midnight
- Saturday and Sunday 8:00am- Midnight (weekend coverage)
- Week 2: OFF
This role is ideal for nurses who enjoy autonomy, flexibility, and being part of a strong, supportive hospice team. The Admissions nurse will complete Admissions only but will be available for admits throughout our entire territory both Mount Vernon and Cassville offices.
Responsibilities
- Complete hospice admissions and start-of-care assessments
- Provide education and emotional support to patients and families
- Coordinate admissions with hospitals, facilities, physicians, and referral sources
- Review eligibility and obtain required documentation
- Communicate with the interdisciplinary team to ensure smooth transitions into hospice care
- Complete timely and accurate documentation in EMR
- Respond to urgent admission needs during scheduled coverage hours
Qualifications
- Current RN license
- Hospice experience preferred, but not required
- Strong assessment and communication skills
- Ability to work independently and manage time effectively
- Compassionate, dependable, and team-oriented
- Reliable transportation and valid driver’s license
