We are seeking a Trauma Nurse Navigator - RN Lead to provide clinical navigation and continuity-of-care support for high-risk trauma patients.
Responsibilities include acting as a consistent point of contact for patients and families, coordinating with trauma surgery, orthopedics, neurosurgery, rehabilitation, behavioral health, social work, care management, payers, and post-acute facilities.
The role requires a Bachelor of Science in Nursing (BSN), RN license, Basic CPR certification, and a minimum of six months of nursing experience. Preferred candidates have experience in trauma, emergency nursing, critical care, or related fields, and supervisory or leadership experience.
Offering a full-time schedule, the position involves a day shift from 7am to 4pm, with no weekends or holidays.
Key duties focus on patient support, care coordination, and collaboration across multidisciplinary teams.
