Role: Nurse Practitioner supporting outpatient memory care within a multidisciplinary team focused on diagnosing and managing cognitive and neurodegenerative disorders such as Alzheimers, mild cognitive impairment, and Lewy body dementia.

- Developing and managing individualized care plans for memory disorders
- Ordering and interpreting diagnostic tests including labs, imaging, and cognitive assessments
- Providing medication management and longitudinal follow-up
- Offering education and support to patients and caregivers
- Collaborating with neurologists, neuropsychologists, and research staff
- Participating in clinical research and innovative care models
- Current Acute Care or Family Nurse Practitioner certification
- DEA license and BLS certification
- Interest or experience in neurology, geriatrics, psychiatry, or cognitive disorders preferred
- Strong communication and teamwork skills
