Key Responsibilities
- Serve as a subject matter expert, improving patient care access and timeliness.
- Collaborate with program leaders to evaluate community needs and enhance navigation programs.
- Provide patient-centered, culturally sensitive care tailored to population specifics.
- Perform assessments, diagnoses, planning, and evaluation, ensuring evidence-based, individualized care.
- Coordinate interdisciplinary communication, documentation, and referrals within specialty clinics and care teams.
- Support performance improvement initiatives and participate in professional development and mentoring.
- Ensure compliance with policies, certifications, and legal standards while maintaining a respectful, inclusive environment.
- Bachelor's or Master's in Nursing preferred.
- Valid RN license; certifications in BLS, ACLS, and disease-specific areas required upon hire, with additional certifications achieved within one year.
- Minimum 5 years of nursing experience, including 2 years in disease-specific and outpatient care or care coordination.
- Strong critical thinking, time management, and emergency response skills in a fast-paced environment.
