- Serve as an expert in patient navigation, improving care timeliness and access.
- Collaborate with leadership to assess community needs and enhance programs.
- Deliver patient-centered care using the nursing process, including assessment, diagnosis, planning, implementation, and evaluation.
- Coordinate multidisciplinary care, ensure documentation, and promote inclusive communication.
- Support performance improvement initiatives and participate in professional development.
- Mentor and precept other nurses, stay current with oncology advances, and uphold organizational policies and safety standards.
- Bachelor’s or Master’s degree in Nursing.
- RN license with required certifications (BLS, ACLS) upon hire; disease-specific certification within one year.
- Minimum 5 years of nursing experience, including 2 years in disease-specific care and outpatient or care coordination preferred.
- Strong critical thinking, time management, and ability to respond decisively in fast-paced environments.
