The Transition House is committed to serving veterans and their families by providing supportive services that promote stability, self-sufficiency, and long-term housing success. We are seeking a compassionate and experienced Lead Case Manager to oversee a team of case managers within the Supportive Services for Veteran Families (SSVF) program.
Key Responsibilities:
Oversee and support a team of 4 Case Managers, reviewing client files before submission.
Conduct monthly supervision and participate in performance evaluations.
Facilitate staff meetings, document minutes, and ensure team compliance with program goals.
Coordinate VA benefits access and integrate benefit plans into individual case plans.
Manage and monitor client eligibility, applications, and progress toward housing stability.
Submit monthly Temporary Financial Assistance (TFA) requests and maintain accurate budget and tracking records.
Facilitate housing placements, including landlord coordination, inspections, and budget management.
Maintain complete and confidential client documentation in HMIS and other required systems.
Administer the HUD Self-Sufficiency Matrix at program entry, quarterly, and at exit.
Attend required trainings and meetings; uphold safety and EEO standards.
- A bachelor's degree in social work or a related field is required, or in nonprofit
- management, or a related field, or (10) ten years' experience in a program leadership
- position in a nonprofit setting; Master's degree preferred
- Extensive organized case management experience through written verification and
- verified documentation
- Intensive case management and experience with families experiencing homelessness
- with children are preferred, and clients with substance abuse and/or mental illness
- Working knowledge of the VA benefits system, including eligibility requirements,
- applications, appeals, VA file documentation, and bilingual is preferred
- Driver's license and dependable private vehicle for frequent travel from site to site with updates car insurance
- Strong interpersonal skills, effective judgment in interviewing clients, and coordinating services with community agencies. Processing all information in a confidential manner
- Good writing skills for program planning, reporting, and correspondence.
- Ability to use independent judgment, perform research, investigations, and inspections, and recommend solutions
- Ability to think and react calmly in emergency situations
