Status: Full Time, Monday- Friday 8:00am-4:30pm, 40 hours, and non-exempt

FUNCTION
Provide trauma-informed case management services to Veteran residents residing at Veterans Way house Shelter. Deliver individualized service plans, housing navigation, life-skills education, programming opportunities, and community referrals in collaboration with the Veterans Housing Program Director and staff to support resident stability and progress.

Responsibilities
ADMINISTRATIVE DUTIES

Resident Engagement & Case Management
Provide direct support and case management to residents, including intake, needs assessments, service planning, and ongoing progress reviews.
Establish, implement, and monitor individualized case plans for all assigned residents.
Conduct regular meetings to assess service progress, address barriers, and update goals.

Housing Navigation & Resource Coordination
Maintain updated knowledge of Lexington public and subsidized housing resources and present options to residents.
Refer residents to appropriate community agencies and encourage engagement with support services.
Assist residents with applications, document gathering, and steps for securing housing, employment, or benefits.

Life Skills, Workshops & Supportive Programming
Present Life Skills classes as needed to support resident independence.
Collaborate with program leadership on skills-building sessions or readiness activities.
Facilitate workshops and information sessions that promote stability and self-sufficiency.
Develop weekly actives such as grocery shopping trips, exercise activities, and group activities.

Follow-Up & Continued Support
Provide limited follow-up services to former residents via phone or home visit when appropriate.

Documentation, Reporting & Accountability
Maintain accurate documentation, case notes, and required paperwork; meet all deadlines.
Complete required statistics and reports on time.
Uphold strict confidentiality in all case-related matters.

Team Collaboration & Community Engagement
Participate in staff meetings, team discussions, and development opportunities.
Attend community meetings or committees as assigned by the Director of VA Housing Program.
Assist colleagues during emergencies or crisis situations.
Refer all admission inquiries to the Director of Veterans Housing Program.
Perform additional duties as needed to support The Salvation Army.
Qualifications
Bachelor’s degree in social work or related field required
Two or more years of social work experience with homeless population preferred
Ability to work in a fast-paced environment
Exceptional written and verbal communication skills
Proficient in Microsoft office
Must possess a thorough understanding and appreciation for The Salvation Army’s mission and work
Must be sensitive to the issues of families, children, cultural diversity, and homelessness- must be able to assist people and aid in a calm, courteous and polite manner
Must maintain a valid drivers license and acceptable motor vehicle record
Must pass extensive background check
