Who We Are
The Resource Exchange (TRE) is a mission-driven organization of 400 professionals serving over 12,000 individuals across El Paso, Pueblo, Teller, and Park counties. We provide person-centered services to individuals with disabilities, mental health challenges, and long-term care needs, focused on independence, inclusion, and collaboration.
Qualifications:Position Overview: CMA Coordinator, Intake
Department: Case Management Operations and Communication
Reports To: Team Supervisor
FLSA Status: Non-Exempt
Starting Pay Range: $22.32 – $23.18/hour (rates; subject to change)
As a CMA Coordinator, Intake, you play a critical role in helping individuals begin their journey toward receiving essential services. You will complete assessments, assist with Medicaid applications, and ensure individuals are connected to the support they need.
What You’ll Do
- Complete and document in-home functional needs assessments (100.2) for HCBS, DI, HCA, and LTHH services
- Assist with Medicaid and waiver applications (including ARG and LTC processes)
- Review and analyze medical, behavioral, and financial documentation
- Maintain accurate and timely documentation across multiple systems
- Ensure compliance with HCPF regulations and HIPAA standards
- Build strong relationships with individuals, families, and community partners
- Manage multiple priorities while meeting deadlines
- Stay informed on community resources to best support individuals
What We’re Looking For
Minimum Requirements:
- Bachelor’s degree in any field
OR - 5 years of experience working with vulnerable populations, in a non-profit setting, or with individuals with disabilities
OR - A combination of education and relevant experience working with vulnerable populations, non-profit organizations, or individuals with disabilities.
Preferred Experience:
- 1–3 years in case management or human services
- Knowledge of Medicaid waiver programs and LTSS
- Experience working with individuals and families
- Familiarity with community resources

Work Environment
- Hybrid role (office, remote, and community-based work)
- Travel to client homes and community locations required
- Must have a valid Colorado driver’s license, reliable transportation, and insurance coverage
- Ability to lift up to 20 pounds and navigate varied environments
