Responsibilities and Purpose
- Identify each resident’s social, emotional and psychological needs.
- Develop and carry out a plan to develop the residents’ full potential during their stay.
- Plan for the resident’s discharge or transition to another care environment.
Experience and Education
- Certificate for Oklahoma-approved Social Services training program preferred.
- Experience in a skilled nursing facility, hospice, or long-term acute care preferred.
- Knowledge of federal and state guidelines.
