Licensed Vocational Nurse - Field Nurse

Job Overview

We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our dynamic team as a Field Nurse. Duties

• Conduct thorough patient assessments including vital signs, physical examinations, and medication reconciliation to inform personalized care plans.
• Administer medications, injections, IV infusions, and other treatments following strict protocols and safety guidelines.
• Perform specialized procedures such as catheter care, tube feeding management, venipuncture, and sterile dressing changes with precision and compassion.
• Document all patient interactions accurately using Electronic Health Record (EHR) systems like Epic or Cerner, Axxess, ensuring compliance with HIPAA and long-term care regulations.
• Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and ongoing treatment strategies for complex cases including dementia, Alzheimer's disease, and other memory care needs.
• Provide education and coaching to patients and their families on medication management, disease processes like infectious diseases or acute pain management, and health promotion strategies.
• Travel within assigned community areas to deliver consistent patient care in homes, assisted living facilities, skilled nursing facilities, or hospitals with Level I or Level II trauma center experience.

Skills

• Valid LVN license with current state licensure required; additional certifications such as Basic Life Support (BLS) preferred.
• Proven experience in critical care environments such as ICU or emergency room settings; hospital medicine or trauma center experience highly valued.
• Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and medical documentation standards.
• Hands-on experience with EMR/EHR systems like Athenahealth or eClinicalWorks; familiarity with Cerner is a plus.
• Expertise in managing complex patient needs including IV infusion therapy, ventilator support, infection control practices, and pain management techniques.
• Ability to work effectively with geriatric populations and individuals with disabilities or developmental challenges such as dementia or behavioral health conditions.
• Excellent assessment skills combined with critical thinking to triage urgent situations accurately and efficiently.
• Exceptional communication skills to educate patients/families and collaborate seamlessly with healthcare teams across multiple settings.

Pay: $24.00 - $26.00 per hour
