We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our team as a Field Nurse, providing vital patient care across diverse community settings. Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews using EMR (Electronic Medical Record)
- Administer medications via injections, IV infusions, catheterizations, and other methods following strict aseptic techniques and HIPAA regulations.
- Perform diagnostic evaluations such as specimen collection and phlebotomy, when required.
- Educate patients and their families on health coaching, medication management, discharge planning, and chronic disease management including infectious disease care.
- Maintain accurate medical documentation of patient care activities and ensure compliance with HIPAA standards.
- Oxygen therapy and handle emergency procedures like CPR or basic life support when necessary.
- Collaborate with multidisciplinary teams to develop personalized care plans aligned with NCQA standards and best practices.
Experience
- Valid LVN license with current state certification required; additional certifications such as Basic Life Support (BLS) preferred.
- Proven experience working in diverse healthcare environments
- Familiarity with EMR/EHR systems such as Cerner, eClinicalWorks, or similar platforms for accurate documentation.
- Prior experience in home & community care, hospice care, or long-term care facilities like nursing homes or assisted living communities is preferred.
- Knowledge of medical terminology essential for effective documentation.
