Key Responsibilities
- Conduct intake assessments and gather client information to determine eligibility for programs
- Manage a caseload and provide ongoing follow-up with clients
- Connect individuals to community resources, including financial assistance and support services
- Document all interactions, case notes, and outcomes in internal systems
- Handle a mix of inbound calls and scheduled follow-ups with a client-first approach
- De-escalate sensitive situations with empathy and professionalism
- Collaborate with internal teams and external agencies to coordinate services
- Bilingual (English/Spanish or English/Creole) required
- 2 years of case management, social services, or community-based support experience
- Experience handling client intake, eligibility, or benefits coordination
- Strong documentation and organizational skills
- Comfortable managing a caseload and multitasking in a fast-paced environment
- Bachelors degree required
- Mission-driven organization making a direct community impact
