We’re seeking a compassionate and dedicated Physical Therapist to join a collaborative care team. This role is perfect for professionals passionate about improving patients’ mobility, functionality, and overall well-being.
Key Responsibilities
Conduct comprehensive evaluations and develop individualized treatment plans
- Deliver evidence-based therapeutic interventions
- Coordinate with other healthcare providers for integrated patient care
- Educate patients and families on exercises and long-term wellness strategies
- Maintain accurate and timely clinical documentation
- Active or eligible Wyoming PT license
- Excellent interpersonal and communication skills
- Ability to work both independently and as part of a team
- Prior home health or outpatient care experience
- Familiarity with WebPT (preferred EMR)
